USING MY LIFE LOG: A Medical Health Diary
Each Section Has Its Purpose
MY LIFE LOG is designed to make recording your vital health data so easy that important information is readily available when you need it most. Using MY LIFE LOG is as easy as one, two, three!
- Fill in the blanks in the front section that pertain to you and your family.
- Go to the second section, THE HEALTH LOG, which serves as an ongoing journal of doctors’ appointments, symptoms, illnesses, prescriptions taken, outpatient procedures, hospital stays, names and information on health care providers and any other health and medical information. (While it’s ideal to start the medical record keeping at birth, it may begin at any point in one’s life.) This format will provide valuable information in the future for both user and health care provider.
- Go to the final section of the book, THE ADDENDUM, which is for overflow writing from the first two sections. Here you add any extra health information that you feel is pertinent.
To Use at Home and Away
Take MY LIFE LOG to all doctor appointments to share important health information that the doctor may find valuable, such as
- Previous treatments
- Surgeries
- Medications you may have or may be taking currently
- Your family’s medical history
Take MY LIFE LOG with you when you travel. The information provided may well save your life. Emergency Medical Information Cards (E.M.I.C.s) are included for your use to provide the first information that is needed in case of emergency, including:
- Current prescriptions
- Drug and food allergies
- Special medical conditions
- The names and contact information of your healthcare providers
and close family or friends
Take the time to read through MY LIFE LOG before making entries. It will be helpful to understand how it is organized. Enjoy using this valuable personal healthcare tool.
