My congratulations on your most valuable book, MY LIFE LOG a Medical and Personal Diary. As an internist, I frequently see patients who are
unprepared to answer important questions about their family’s health history such. Even remembering when symptoms first began and what prescriptions
are being taken is difficult for many patients to remember. If patients would
enter the appropriate information and bring their copy of MY LIFE LOG with
them to appointments, it would be a great help. It’s a way that the patient can
participate in making sure they get the most from their appointment with the
doctor.

Your book is an excellent system for keeping personal health information
and is full of sound medical facts. I like that you provide Emergency
Medical Information Cards which everyone should carry with them at all
times.

What an important document to share with future generations due to the thousands of genetically traceable conditions.

My wife of more than 40 years started to experience some nagging back problems and pain that led to us seeing five different back doctors. There were X-rays, scans and a host of other lab tests. Every doctor wanted a complete history. Within a few months she was on seven new medications and each new doctor wanted to change something. I found myself in information overload. I started to keep a computer file of all the facts that included doctors’ names, addresses, phone numbers, appointments, prescriptions, lab results, treatment history, and more.

Within a year of her apparent recovery we moved to another state and new doctors. I was able to provide the new doctors with concise overviews of previous treatment.

Neurological tumors developed that required surgeries. In the middle of all this, our health insurance company lost records and hospital and doctor bills were not getting paid. Fortunately, I had written records on my computer to help sort all the payments out. This took months but was finally resolved.

I improvised the recordkeeping as needed. Unfortunately, it went on for years. Thank you for creating a document that provides for recording health and medical information. It’s such a valuable concept. We all need to have this information available for new doctors, for travel emergencies, and for our families.

Thank you for creating MY LIFE LOG Book!

I am very excited to see this book published. Judy and I have spoken about what this book would offer. Little did I know how important this would become in my own life. Last summer, my 76-year-old father came to live with me.  He was in fairly good health; he had some medications he took daily, but because of Judy’s book, I thought I better write down all of my dad’s medical needs; his doctors, medications, appointments, and so forth…Not even a week later, my dad was rushed to the hospital.  I grabbed my make-shift medical log and took it with us.  When the nurse asked my father for his history, he could not answer because he was in so much pain. Luckily, I had it! The nurse took all his information down and was thrilled that it was all written in a notebook. If I had not heard about Judy’s MY LIFE LOG, I would have struggled with the knowledge of my dad’s medical history.  I know the book has so much more to offer and can’t wait to purchase one for each member of my family.   I believe anyone you care for should have one.
